Job Description
The Case Coordinator will conducts a comprehensive assessment of patient's social service needs, in compliance with the requirements of the policies of Health Home.
Case management duties will include : global evaluation and identification of patient's social service, entitlement and home service needs;
development, implementation and documentation of a service plan for each patient.

- Bachelor's Degree
- Minimum of 1-3 years of experience in relevant case management service delivery
